Description: The Assessment Tool is a series of questions which will help determine if you pre-qualify for a recommendation for medical marijuana. Once you pre-qualify, call our office for a required face-to-face medical marijuana evaluation appointment with the doctor. Once the face-to-face medical marijuana evaluation is completed the doctor may issue a recommendation for Medical Marijuana . You will receive a Physician Statement (Letter size and Wallet size) and a Medical Marijuana Identification Card (MMIC). This will allow you to legally possess, use and grow medical marijuana for medicinal purposes.
